NYeC has worked with providers for many years to assist them in their journeys toward value-based care (VBC). Support in these areas from NYeC has and can take place on-site or remotely, as direct assistance and/or education and training.

Patient-Centered Medical Home (PCMH)

Patient-Centered Medical Home (PCMH) is the foundation for achieving high-quality, accessible, efficient health care. To earn PCMH Recognition, practices must meet rigorous standards for addressing patient needs.

With expertise in PCMH concepts and navigating the PCMH Recognition process, the NYeC team is equipped to analyze current practice workflows, policies, and data to identify gaps and develop a plan to progress through each PCMH virtual check-in meeting.

NYeC brings proficiency in creating internal policies and procedures and new workflows to meet PCMH requirements combined with support in the form of personalized training, skilled coaching, and guidance, and technical assistance will transform your practice into a Patient-Centered Medical Home to ensure your practice sustains PCMH Recognition through annual reporting.

 

NYS Patient-Centered Medical Home

As part of ongoing initiatives within the State and the broader industry to transform healthcare delivery and shift towards VBC, NYeC managed a group of Transformation Agents (TAs) on behalf of the NYSDOH to help participating practices achieve and maintain certification for the NYS Patient-Centered Medical Home (NYSPCMH) Initiative. This program aligned with the Quadruple Aim of improving patient care, improving population health, reducing healthcare costs, and reducing provider burden.

175 practices worked with NYeC to achieve and maintain NYSPCMH recognition. They received a complete practice needs assessment and evaluation to identify gaps and map out a work plan designed to get practices prepared for value-based care (VBC).

NYeC then worked with the partnering TAs to:

  • Create Customized Curriculum
  • Provide Expert Guidance And Support
  • Deliver Best Practice Training,
  • Successfully Implement Workflow Changes Designed To Strengthen The Clinician-Patient Relationship While Improving The Quality Of Care Within Participating Practices.

 

Between 2015-2019, NYeC was one of the leading Practice Transformation Networks (PTNs) through the CMS Transforming Clinical Practices Initiative (TCPI). Through this program, NYeC helped practices in New York State (NYS) demonstrate cost savings, improve health outcomes, reduce unnecessary hospitalizations, and assisted practices in joining an Alternative Payment Model (APM). Through work with strategic partners located across NYS, some significant successes in the TCPI program included more than 3,800 clinicians joining an APM and avoided more than 27,000 unnecessary hospitalizations over five years.
